Three Ways to Enroll:

1. Visit www.coveredca.com to download the application and enroll in LIBERTY's family or pediatric dental plans through Covered California for Small Business.
2. Enroll through an agent or broker.  Refer to https://v.calheers.ca.gov/hix/broker/search/individual for a listing of Certified Insurance Agents in Covered California. 
3. To enroll directly through LIBERTY outside of Covered California for Small Business, submit a request through the Contact Us form.
