Co-Pays
Please review your dental benefits carefully. Most of the treatments have a $0 co-pay. However, there are some treatments that may require a co-pay from you. Before you accept a treatment with a co-pay, be sure you know what you will need to pay out of pocket. Your dentist can answer any questions you have about the co-pays and tell you about the options you have. Once you agree to a treatment with a co-pay, Alliance CompleteCare will not cover any out of pocket co-pays for you.
Missed Appointments
LIBERTY Dental Plan charges an $25 co-pay for missed dental appointments. If you scheduled a visit to your dentist and cannot make it, you must let the dentist know 24-hours in advance. You are responsible for the co-pay for missed appointments. Alliance CompleteCare cannot cover the co-pay for a missed visit.
Free Interpreter Services
If you prefer to speak in a language other than English, Alliance CompleteCare can schedule an interpreter for your visit. This is a free service offered by the Alliance. To schedule an interpreter, please contact your Alliance Care Advisor at 1-877-585-7526 (TTY 1-888-747-1574) 8am - 8pm, 7 days a week.
