LIBERTY Dental Plan is a privately held company with no affiliations with other health plans. Dental benefits are LIBERTY Dental Plan’s only business.
D-HMO plans for Individual and families, Reduced Fee-for-Service Plans, PPO and traditional indemnity dental programs are provided to employer groups, associations, Labor Unions, Taft Hartley Trust Funds, municipalities and government sponsored programs (Medicaid).

How do I receive care?
You must choose a Primary Care dentist when you enroll in the plan. Please use our Find a Dentist link above.
You are free to choose any Primary Care Dentist off of our directory. You and your entire family must use the same dentist. You may change your dentist if you it is necessary. Once you have selected a dentist, or if you have any questions about your selection, please contact our Online Support Department at (888) 273-2719 between the hours of 8:00am to 5:00pm Monday through Friday.
How do I make an appointment?
After completing your enrollment form and selecting your Primary Care Dentist, you are eligible to receive care within 48 hours following LIBERTY's receipt of your enrollment application and premium.
Please note that you must select a Primary Care Dentist and submit your selection to LIBERTY before you call the dentist to make an appointment. You may call your selected dentist at any time after the effective date of your coverage, typically 48 hours following receipt of your application and premium.
Be sure to identify yourself as a member of LIBERTY Dental Plan when you call the dentist for an appointment. We also suggest that you keep you Evidence of Coverage and Schedule of Benefits handy when you go to your appointment. You can then reference benefits and applicable co-payments which are the out-of-pocket costs associated with your plan.
