Renewing Your LIBERTY Dental Plan?
It’s fast, simple, and secure. Renew your Plan, Upgrade your Plan, or change your membership by choosing any one of the three plans below. Adding or deleting dependents, or changing your address can be completed in the renewal application. If your plan expired more than 6 months ago you will be required to pay the $20.00 Enrollment FEE included in a New Membership Premium. If you have any questions, please call (888) 273-2719 Mon- Fri between the hours of 8AM and 5PM. Your renewal Activation will be sent to you by email within 24 to 48 hours.
