LIBERTY Dental plan has designed a Dental plan specifically for Southwest Carpenters members and their families. With free cleanings, x-rays and low cost exams, an Individual can save over $300 on their initial visit. After paying for the premium you still save over $150.

*A Primary Care Dentist will be assigned to you and your family when enrolling in the CA50 Plan. If you wish to change to another LIBERTY Dental Plan contracted provider, simply contact our Member Services Department (888-703-6999) by the 20th day of any month and the change will be effective the first day of the following month.Those enrolling in Plan CA 80 do not need to select a Primary Care Dentist at the point of enrollment. To access care with the CA80 Plan, simply contact a LIBERTY Dental Plan provider who is contracted to provide services under your selected Plan for an appointment.
