When you first enroll in our plan, you will select one of our Network Primary Care Dentists for you and your covered dependents selecting from a wide network of private dental offices. If you do not select a Primary care Dentist, one will be assigned for you based on your home zip code. For coverage under this plan, you must obtain services from your assigned Primary Care Dentist. You may change your Primary Care Dentist by calling our Member Services Department at (888) 703-6999. Primary Care Dentist Change requests received by the 20th day of any month will be effective the first day of the following month.
