For coverage under this plan, you may use any contracted LIBERTY Dental PLAN EPO Primary Care Dentist.  LIBERTY Dental plan contracts with a wide network of private dental offices to provide dental services under this plan.  Primary Care Dentist pre-assignment is not necessary; however, you must obtain services from an contracted LIBERTY Dental Plan EPO Primary Care Dentist to have coverage under this plan.
