To obtain dental services, simply contact any LIBERTY Dental Plan contracted Cobalt Plus provider to schedule an appointment. The dental office will then call us to verify your eligibility in the plan. You will only have coverage for services received from a provider who is a contracted LIBERTY Dental Plan Cobalt Plus provider.
