County of Kern - FOR PPO MEMBERS

You are free to choose any licensed dentist for treatment, but it is to your advantage to choose a PPO network dentist. PPO contracted dentists have agreed to an amount per covered procedure of which the plan pays 90%. Non PPO dentists' services are covered at 70% of the regional customary charges (subject to a maximum allowable charge). You are responsible for your co-insurance percentage (based on the Plan) of the agreed upon amount, deductibles, non-covered services and any amount over the allowable maximum charges.

Your annual deductible met to date and amounts paid by the prior administrator will be applied to your LIBERTY Independence PPO coverage. Your deductibles and maximums will NOT start over on April 1, 2008.

If you or your dentist has a pre-treatment benefit estimate for services that are not performed prior to April 1, 2008, please have your dentist re-submit the estimate request to LIBERTY Dental Plan. We will work with you and your dentist to get it processed as soon as possible.
