Notice to Members Impacted by fires in Butte, Los Angeles and Ventura Counties

If you reside in Butte, Los Angeles or Ventura County and you’ve been affected by the Camp, Hill or Woolsey fires, please read this important message.

On November 8, 2018 Acting Governor Gavin Newsom declared a state of emergency in Butte, Los Angeles and Ventura Counties due to the effects of the Camp, Hill and Woolsey fires. If you are a LIBERTY Dental Plan member, and you have been impacted by the effects of the Camp, Hill or Woolsey fires, we may be able to help by:

  • Relaxing time limits for prior authorizations or referrals.
  • Suspending prescription refill limitations and permitting impacted enrollees to refill their prescriptions at out-of-network pharmacies.
  • Extending filing deadlines for claims.
  • Allowing access to appropriate out-of-network providers if in-network providers are unavailable due to the fires or if you are out of the area due to displacement as a result of the fires.
  • Providing toll free telephone numbers that affected enrollees can call for answers to questions, including questions about the loss of ID cards, access to prescription refills, or how to access health care.
