LIBERTY DENTAL PLAN OF FLORIDA -- HURRICANE DORIAN INFORMATION (UPDATED: 8-30-19; 10:18 AM EST)

In response to Hurricane Dorian, Florida’s Governor Ron DeSantis issued Executive Order 19-189 and Executive Order 19-190 declaring a state of emergency in all 67 counties in Florida.

If you or your child are a Medicaid enrollee enrolled with LIBERTY Dental Plan, pursuant to the Governor’s Executive Orders, LIBERTY will:

  1. Furnish covered dental services to our Medicaid enrollees without any form of authorization, without regard to whether such services are provided by a participating or non¬participating provider, and without regard to service limitations; and,
  2. Implement a readily-available claims payment process to ensure dental providers are paid for services rendered before, during, and after the disaster, as medically necessary.

If you need assistance from LIBERTY Dental Plan as a result of Hurricane Dorian, please call (877) 869-1803.
