LIBERTY's Family Dental HMO Plan through Covered California

LIBERTY Dental Plan is proud to bring an affordable family dental HMO plan for adults and children through California's Healthcare Exchange Marketplace. LIBERTY recognizes and shares in the Covered CA's commitment to quality, affordability, and service.

How to Enroll

  1. All Californians can sign up during the annual open-enrollment period from November 1st to January 15th. Many others may be eligible to sign up at any time during the year due to a life-changing event such as getting married, having a child or moving.
  2. To apply directly with Covered California, visit www.coveredca.com and click 'Apply for Coverage' to enroll in LIBERTY's Family Dental HMO plan.If you need assistance, refer to Covered California’s listing of Certified Insurance Agents.
  3. LIBERTY offers individual plans outside of Covered California that are available throughout the year. To enroll in LIBERTY's CA50, CA80 or CA90 plans (outside of Covered California), visit our California Individual Plans website.

Who to Contact

Contact Covered California at (800) 300-1506 or visit CoveredCA.com to:

  • Update your address and contact information.
  • Report any changes to your income
  • Update information such as citizenship and proof of residency
  • Make changes to your health coverage
  • Ask questions about financial assistance
  • Cancel your coverage
  • Request a copy of your Form 1095-A

What is Covered California

Covered California (Covered CA) is a part of the Health Insurance Marketplace created by the Affordable Care Act (ACA). Covered CA offers individuals a choice of qualified health and dental plans and tools for making informed choices. Eligible individuals have access to federal premium assistance. You will use the Covered California website to apply for coverage, compare plans, and enroll.

Why Pediatric Dental is Required

The Affordable Care Act (ACA) requires pediatric oral benefits as part of the 10 essential health benefits offered by medical plans to individuals and small employers. In California, all Covered CA health plans automatically include embedded pediatric dental benefits, which only provide dental benefits for family members up to age 19. Only standalone dental plans offer dental coverage for both children and adults and are purchased separately.
