LIBERTY has administered the Los Angeles Pre-Paid Health Plan (LA PHP) program since 2005. We have a network of about 1,175 unique providers or 2,721 points of access (general dentists and specialists) servicing over 500,000 members in Los Angeles County.

LIBERTY now pays higher reimbursements in the Managed Care Program as well as the Denti-Cal Program. For more information on the fee schedule or how to enroll in our network, please complete our provider interest form or contact our Network Manager at 800-268-9012.
