Completion of the Critical Incident Awareness Training Program will meet the one-time training requirement for this topic as required by the Center for Medicare and Medicaid Services (CMS). The goal of this training is for providers to recognize reportable critical incidents and to learn how to report them.

This attestation form is provided as a convenience for contracted Providers and Staff to verify that they have completed the entire Critical Incident Awareness Training Program.
