Liberty Dental Plan (“Liberty”) would like to hear from you. Once every three months, we meet to discuss how we are doing and how we can improve. The committee is made up of members, Liberty’s Dental Director and other support staff.

You will be part of:

• Suggesting ways to improve programs and services
• Reviewing reports on the Plan’s quality metrics (service measures)
• Helping to set policies that affect you

You will receive payment for every eligible meeting your attend.

• The committee will meet once (1) per quarter and four (4) times per year.
• Members will be compensated $50 per quarter and no more than $200 per year.
